CLOUDCARE: POPULATION HEALTH MANAGEMENT SYSTEM FOR DIABETES

Innovations in diabetes technology have dramatically improved outcomes of persons with diabetes (PWDs), but have increased healthcare provider (HCP) workload due to high volumes of data. CloudCare is a population health management solution continuously overseeing data from larger groups of PWDs simultaneously. Based on remote monitoring, screening and triaging, it enables identification of PWDs with deviating glucose values, potentially leading to decreased HCP workload. This ongoing observational study is assessing the effect of CloudCare on treatment satisfaction, contacts between HCPs and PWDs, and glycemic control.

 

A 6-month interim analysis was performed among 175 adult PWDs diagnosed >6 months ago. The primary outcome was PWD treatment satisfaction (DTSQ). Secondary outcomes included number and type of HCP-PWD contacts (per PWD per 3 months), diabetes-related distress (PAID-5), and glucometrics (GMI, TIR, TAR and TBR). The results were presented as a poster at ATTD 2025 and published as an article in BMC Endocrine Disorders.

Key findings:

  • Age and diabetes duration were of 34 years and 20 years, respectively
  • Automated insulin delivery was used by 95% and MDI+CGM by 5%
  • Treatment satisfaction change was +7 (minimum is ‐18, maximum is +18; > 0 is increase in treatment satisfaction)
  • Number of contacts significantly decreased, mainly due to decreased face-to-face contacts
  • The decrease in diabetes-related distress was lower at both 3 and 6 months, vs baseline
  • Glucometrics did not significantly change

 

Concluding, the authors state

"Application of CloudCare in this cohort decreases the workload for healthcare professionals, while increasing PWDs treatment satisfaction and maintaining excellent glycemic control. CloudCare therefore has the potential to change care pathways, use data from diabetes devices constantly and more appropriately and help to address the increasing shortage in healthcare resources. " -
